In accepting a grant of $_______________from The Joanna Foundation ("Foundation"),

gives assurance of the following:

1.	Confirms that the Grantee is exempt from federal income taxes under Section 501 (c) (3) of the Internal Revenue Code and is not a private foundation under Section 509 of the Internal Revenue Code.

2.	Agrees to notify the Foundation immediately should the Internal Revenue Service revoke the Grantee's exemption or publish Grantee's name as an organization not exempt under Section 501 (c) (3).

3.	Agrees to expend the funds received from the Foundation only for the purposes of the grant, for the public good, without discrimination on the basis of race, creed, sex, religion or national origin, and to refrain from using any of the funds for the purpose of lobbying or engaging in political activity.

4.	Acknowledges that, in making this grant, the Foundation does not guarantee or warrant in any way to anyone any product or service acquired or used by or rendered for or in consideration of the Grantee's use of these funds.

The acceptance of this grant and the terms and conditions stated above is signed by an authorized officer of the Grantee.
