
Applicants should use The Joanna Foundation application form.
The completed application form should be signed by the
applicant organization's executive officer or board
president. Only one proposal per organization will be reviewed
in a calendar year.
Organizations approved for funding
will be asked to complete and return a statement of acceptance
to activate the grant.
Requests
for funding should include all of the
following
information:

- Basic needs and objectives of the request
- Impact within one or more of the five designated counties
- Brief description of how the project or program will
be implemented timing, duration, staff, facilities,

- Plan for sustainability or ongoing support

- Documentation of federal tax-exempt status

proposals may not be reviewed. No materials will be returned
