Applicants should use The Joanna Foundation application form. The completed application form should be signed by the applicant organization's executive officer or board president. Only one proposal per organization will be reviewed in a calendar year.

Organizations approved for funding will be asked to complete and return a statement of acceptance to activate the grant.

Requests for funding should include all of the following information:

1. Completed, signed application form
2. Program information (no more than 4 pages)
 • Brief history of the organization
 • Basic needs and objectives of the request
 • Brief description of how the project or program will be implemented    (timing, duration, staff, facilities, etc.)
 • How effectiveness or outcomes will be evaluated
 • Plan for sustainability or ongoing support
3. Organizational information
• Name and primary affiliation of officers and board members
• Documentation of federal tax-exempt status
4. Financial information
• Organization’s budget for the current year
• Year-to-date revenues and expenditures
• Project budget (if applicable), indicating total costs, use of requested
  funds, and other sources of support
• Organization's financial statements (preferably audited) for the most   recent fiscal year

If your organization has received a grant from The Joanna Foundation that is designated as renewable, please use the Request for Continuation form below to submit your request for renewal.

An original plus seven (total of 8) copies of all materials are requested. Incomplete proposals may not be reviewed. No materials will be returned to the applicant. Applications are not accepted electronically. Proposals should be mailed to The Joanna Foundation, P.O. Box 308, Sullivan's Island SC 29482-0308. The trustees review proposals three times a year. Proposal receipt deadlines are January 15, May 15, and October 15. Eligible requests postmarked by the respective deadline date will be considered at the next meeting. Applicants will be notified within six to eight weeks of the deadline date.
